A root cause analysis in video format, produced by the Lyondell Basell refinery, has been posted on YouTube by a U.S. heavy-lift and transport planning specialist. The analysis, which according to the poster is Part One, was produced after the July 18, 2008 VersaCrane TC36000 collapse that killed four contract workers at Lyondell Basell refinery in Houston, Texas. The intention, according to the video, was to analyze what went wrong and prevent a similar situation from occurring again in the future.
